FACILITY IS HEREBY ALLOWED TO OPERATE WITH PERMISSION FROM THE KANKAKEE COUNTY HEALTH DEPARTMENT.
FOLLOWING ITEMS TO BE ADDRESSED: 1)CLEAN AND SANITIZE ALL EQUIPMENT AND SURFACES BEFORE OPENING 2) REMOVE AND CLEAN UP AND UNNECESSARY ITEMS FROM THE FACILITY INSIDE AND OUT. 3) ENSURE THAT THE ESTABLISHMENT KEEP HAS THEIR OWN SUPPLY OF TEST STRIPS. SHARING RIGHT NOW WITH THE RESTURANT. 4)ENSURE THAT THE CEILING BY THE WALK-IN COOLER AND IN THE WOMEN'S RESTROOM IS IN GOOD REPAIR. 5) MAKE SURE THE NEW BACK FLOW PREVENTION DEVICE FOR THE ICE MACHINE GETS INSTALLED WHEN IT GETS DELIVERED. 6) ENSURE ALL EMPLOYEE RECEIVE THEIR FOOD HANDLERS TRAINING AS WELL AS THE FOODBORNE ILLNESS AWARENESS TRAINING. ITEMS WILL BE CHECKED DURING THE FIRST FULL ROUTINE INSPECTION. RECEIVED $250 FOR THE FOOD LICENSE APPLICATION PERMIT. |
